Parental Consent Form - Kigumi Youth Advisory Panel

Purpose of Consent

I, the undersigned, hereby grant permission for my child to participate in the Youth Advisory Panel, a pro bono, non-commercial activity organized by Kigumi Group Ltd. for adolescents globally.

Scope of Consent

This consent covers my child’s participation in Kigumi Group’s Youth Advisory Panel.

Risks and Responsibilities

I acknowledge that participation in this event/activity involves inherent risks. I understand that Kigumi Group Ltd. will take reasonable precautions to ensure the safety and well-being of my child, but I accept ultimate responsibility for my child's safety and conduct.

Data Protection

I understand that my child's personal data will be collected and used solely for the purposes of this event/activity, in compliance with Hong Kong's Personal Data (Privacy) Ordinance (Cap. 486). I understand that my child’s messages and texts to the group chats as part of the Youth Advisory Panel may be used as anonymised testimonies in Kigumi Group’s work with students on AI safety and digital wellbeing.

Duration of Consent

This consent is valid from 1 January 2026 to 1 January 2027.

Right to Withdraw Consent

I understand that I may withdraw this consent at any time by notifying Kigumi Group Ltd. in writing at info@kigumigroup.com.

Declaration

I declare that I am the parent/legal guardian of the above-named minor and have the legal authority to provide this consent. I have read, understood, and agree to the terms outlined in this form.
